OHID

To be assigned a role in OH|ID, you first must have an OH|ID account and Profile created.  If you do not have an account, you can do so here.  https://ohid.ohio.gov/create-account

You will need to have your driver's license and an email address to complete the process.

Please include the following information:

  • District Assignment/Role/Job Title
  • District Location
  • Name of supervisor
  • Has your supervisor approved this request?
  • Legal name used in the OH|ID application
  • Educator State ID or date of birth
  • Name of requested role or if you do not know the name briefly describe the information you are looking for
    • When assuming a position previously held by another person with the same role, please include their name
