You will need to have your driver's license and an email address to complete the process.

- District Assignment/Role/Job Title
- District Location
- Name of supervisor
- Has your supervisor approved this request?
- Legal name used in the OH|ID application
- Educator State ID or date of birth
- Name of requested role or if you do not know the name briefly describe the information you are looking for
- When assuming a position previously held by another person with the same role, please include their name
